Crescent City Physical Therapy staff are all highly educated and experienced. All staff have attained advanced educational degrees, advanced continuing education, and specialize in the treatment of patients with orthopaedic injuries, disease and disabilities.

Referring a patient to Crescent City Physical Therapy is as easy as 1 - 2 - 3!

1. Contact any one of our offices via telephone or fax.

2. Our office will interview all newly referred patients for insurance information and we will secure insurance verification and pre-authorization for therapy services. Patients will then be scheduled at the time and location most convenient for the patient. Our goal is to initiate therapy within 24 hours following the insurance verification and pre-authorization process.

3. Patients will be evaluated for physical or occupational therapy and treated per physician referral. The Plan of Care and follow up reports will be communicated to the referring physician.
